Manure is fine as far as smells are concerned but questing in Angmar would claim many casualties with the poisonous air and what not.

Well, Turbine tried that in Asheron's Call, sort of.
In the various Allegiance Halls (there's one in or near every city, and they're all built to the same plan), there's a side room that looks as if it's intended to be a stable (though there are no mounts of any kind in AC), and there's a drain in the floor. If you stand over the drain for a certain number of seconds, you get damage and the message that the noxious vapors from the drain are making you sick.
