What is wrong with paying attention to what you are doing?
Most people will get right every time. The majority of the folks that do it wrong once - will not make that mistake again.
I've worked with UI design over the years. People do not pay any attention to "yes" or "no" dialogues. They almost always (99%) of the time go Grr and hit Yes. The 1% typically go Grr - I hit No. Now I have to start over. I hate the Bunny.
IMHO - You never want to use a confirmation panel unless the "Yes" results in:
1) Some sort of action that can't be undone. Like shredding a file.
2) In the loss of some real or virtual item(s). Like destroying an item.
In summary, the careful folks do not make these mistakes. The non careful, can't read, don't know what they want will still make mistakes. It annoys everyone without improving anything.

Why Would this be needed?
You clicked on the options menu to open it.
Why would you click Exit if you wanted to adjust UI Settings or Get Help
Since you have to click on options to get to the Exit game buttton that is simillar to a confirmation as it is a multi step function.
There are 4 buttons and a return to Game button Paying attention to choices alone solves the problem

Doesn't one of the F-for-Function keys do an automatic drop from game? I seem to remember reading that there was. (It won't be F1 through F6, which selects your fellows, nor F9 through F12, which have various known functions. I suppose I could log in and try F7 and F8 and see what happens, but it's late at night and I'm sleepy. Anyone know, offhand?
Anyway, if it's one of those, maybe the OP is talking about that.

A colleague of mine once programmed a confirmbox with a randomly placed ok-button, just so to get rid of the complaints of people not reading and then not happy about the consequences ..
but even that didnt really help, as well as triple confirmboxes..
best thing probably would be requiring you to type in a word highlighted in the text above :-)
It probably is best to reduce the amount of user interactions, to save clickies for then they are really necessary (and then get the right amount of attention) ,so just as you said, #1 and #2.
I've designed UIs for a living too, and one of my pet-peeves is unnecessary confirmation dialogs. I greatly dislike them. Sure, it's necessary to have a confirmation if you're about to delete your character. But just to exit the game? No. As Yula said, many people won't ever make that mistake, and those who do will quite quickly learn not to repeat it. Why make every single player have to deal with an extra confirmation simply to exit? When I press the "X", I want to leave.
Speaking of unnecessary confirmations, why hasn't Turbine fixed that annoying step where I have to press "Next" when I only have a single subscription to the game? Come on. They said it was a bug like 3 months ago. How hard can it possibly be to add that check in the launcher?

The "M" toggle works great unless you are a chatty person like myself in which case you end up with an M in the middle of what you are saying rather than closing the map.
Now, I do actually use the M key MOST of the time.... But um... Yeah. I can't tell you how many times I have closed the game instead of the map. It's in the hundreds by now.

I believe the problem the OP is trying to address is one that occurs when you're button-mashing escape to close out windows (due to aggro or whatever reason you need to clear the screen completely and quickly) and then your main menu pops up with the 'Exit Game' option conveniently directly in the middle of the screen, where it's quite easy to accidentally click on it while trying to click on something else (a mob or interactive object in an instance, for example).
Accidentally clicking that is not out of bad habit or carelessness, it's just bad implementation, like how if you're attempting to type something and someone sends you a trade/fellowship invite you're suddenly opening every window on your UI because you don't have enough time to react to it knocking you out of the chat window, so you're hitting hotkeys.
I think adding a 'yes/no' confirmation would be useful, as there is already a 30 second countdown to logging out regularly with an 'exit now' or cancel option. The way the exit game instantly boots you out could easily cause trouble in various situations, especially if you're a vital member of your group, like a tank or healer.
It is weird that for logging off you get a confirmation + timer but for exiting the game altogether (which is much worse when done accidentally) there is nothing.
What Yula says makes sense though, there shouldn't be too many confirmation dialogues flying around. If you only got one when doing something very serious and critical then it would get your attention instantly. But if you had to confirm everything all the time, then it would just be annoying and you would hit yes automatically without thinking even when doing something critial.
People who want a confirmation for quitting the game are way, way off and I certainly don't want a confirmation.
There has to be a quick way of exiting a game, because if you are playing Lord of the Rings Online while at work you simply have to be able to close the game down immediately so that your boss doesn't notice what you're doing.

Is it possible you're referring to Alt-F4?
I recently rediscovered that that's a way to immediately exit the game... I was looking for the personal night-light (Alt-F10) and knew it was Alt-<some function key>... I also knew that there was something about Alt-F4... yeah... the 'something' was that it immediately closes any Windows application that respects that combination. Whoops!

We had an internal discussion years ago about confirmation boxes and the text in the window. I keep pushing the concept that people never (rarely) read the text. Hit Yes to confinue. We did a series of tests with different colors, reversing the question. My favorite was the "claim the reward" in the information text. We had a way to easily change the text for all internal users - other developers and the testers. We got very few takers or comments back. Even when the prize was something decent like a free lunch.
Somebody else mentioned this issue. Many cases there are reasons why customers want to get it done quickly. Perhaps their lover (Ms. Yula) is a NOT a gamer Grrl. She is home early. I going to lose Wife faction points. I need the client closed now. So that the window behind the client which has Ms. Yula approved content on it is in focus. Ms. Yula one of those, I turn off my phone. I do not respond to messages. I will be home sometime between 5-11 PM.
The X in the upper right. Microsoft has been doing that with apps since Windows 3.1 time frame. It is something you have to get used when you have an application window. Do not click on the X unless you want to kill it. My complaint is that minimize and maximize especially are so close to the X that is easy to make a mouse pointer and click error. I wish there is a safety zone maximize and exit.

That's not really a confirmation. That's a "leave now" vs. "cancel action" choice. The alternative being to do nothing and log the character out at the end of the countdown.

Agreed 100%. This is when I have the issue. I've also exited accidently when wanting to logout. Even just placing the "exit" button away from everything else, for example at the very bottom of the box would make me happy.
Just move the button a little bit.

I have a personal interest in this suggestion, but agree that further confirmation is not necessary as it already exists, but only when we are using the product "not as intended" does it become an issue (Window Mode clicking error).
Dingador, why I say this is because a while back the Map had a different mechanic that made it possible to close the game. In Map mode, the screen had an X at the upper right corner. Out of habbit, one could click the X to close the screen but the entire game shut down. No confirmation, no secondary screens up, nothing. Bam! Game closed. I complained in the Forums and didn't get much support and actually Yula chewed me out on that one as well () for being lazy, but it was within the game, using a game screen, no Window Mode, just another screen like the crafting panel, etc. It was changed and I was happy.
So from my perspective, I like the way it is. In game menu, Options, Quit...Do you want to Quit? Yes or No. It's perfect.

Edit: In Window Mode you are more in the Task management scenerio and a screen should be closable using the X. What I mean about the old Map is when fully in Game Mode, the Map used to have an X. That shut down the entire screen, no warning.

Doesn't matter. Why? Because the computers at my work would explode in protest if I tried to play a graphic intensive game on them. They might be able to handle Oregon Trail!
