    Totally never been done. Game is only 5 years old.

    Plenty of non-stealth classes have 5 stars, i myself have 4 creeps at 2k rating, 3 of them being visible classes.

    Its not terribly uncommon.

    I've gotten 5 stars on my WL twice in the past year. He is now currently still sitting on 5 stars.
