I believe it's in combat as well. Doesn't say out of combat run speed like most things. 5% isn't that much, so it's not particularly noticeable. There's no icon because it's a passive, I believe.

Urugarth and Carn Dum aren't terribly challenging. They're better than the Rift and Helegrod raid. Plus, most everyone runs CD and Uru at some point because that's where the major class items are...

The fear affects anyone in melee range. So hunters and minstrels and RKs can stay far enough back that they won't be hit by the debuff, leaving fewer people in need of a fear pot of Muster Courage.
