When I did it on my champ, I had to go ahead and pull the next wave before Lorniel ran up to them so I would gain aggro first. It was tough but you should be able to do it on a guard

There is no blocked entrance to the Great River region. The only two ways to get there are from Lothlorien and Rohan. The blocked entrance is probably going to be the future entrance to Western Rohan...

All players can go to all quest areas whether they are F2P or not, however you would need the Angmar quest pack to complete quests in Angmar (besides the epic storyline).

Some of the higher populations servers include Brandywine, Landroval, Elendilmir, and Riddermark. I know that Brandywine and Riddermark are not RP but I'm not so sure about Elendilmir and Landroval....
